Lockout tag out (Energy Isolation)

A. While repair or other work being performed on equipment must be locked out as defined by the Permit to work and Isolation Certificate

B. If a lockout provision does not exist on the equipment, the equipment must be properly disconnected from the power source or otherwise deactivated, unless the nature of the work being performed necessitates that the power is connected or the equipment activated. This requires approval from the OIM.

C. A warning tag must be placed at the point where the equipment connects to a power source and at the location of the control panel activating the power. 


The warning tag must state:

1. That the equipment is being worked on, and if the power source is disconnected or the equipment deactivated, the power source must not be connected or the equipment activated.

2. The name of the person placing the tag. 


Lock out-tag out

1. The tags must not be removed without the permission of the person who placed the tags, that person‘s immediate supervisor, to their respective relief.

2. It is the responsibility of the OIM, Toolpusher, or Barge Engineer to provide padlocks (if appropriate) and tag out tags. It is also their responsibility to assure that this rule is enforced.

3. Any electrician equipment that has been locked out or tagged out must be done under the authority of a Permit To Work and an Isolation Certificate and have a log entry by the electrician in the “Electrician’s log book”
